The incident occurred during a high-profile domestic league match attended by around 40,000 spectators.
Trouble reportedly began after high-powered fireworks were set off in the section allocated to Cerro Porteno fans. Security forces said the devices had been smuggled into the stadium.
Eyewitness accounts also described chaotic scenes, including fans seizing a riot shield from a police officer and displaying it in the stands.
Security forces responded with tear gas and rubber bullets, prompting panic in the stands. Many spectators moved onto the pitch as smoke filled parts of the stadium.
The match was abandoned after 29 minutes of play, and around 100 people were detained.
Security officials confirmed that at least six police officers were injured, including one in serious condition. Hospital representatives said injuries included head trauma and lacerations.
Authorities said they are working to identify those responsible for triggering the violence and may impose bans from future sporting events.
The Paraguayan Football Association states that a team may forfeit a match if its supporters cause a suspension.
Olimpia has indicated it will request the points for the abandoned match, while Cerro Porteno has argued that responsibility for stadium security lies with the organising club.
Olimpia currently leads the national league standings, with Cerro Porteno in second place.
